Puffy, localized irritation on scrotum of long duration

Not sure if this is a growth or a rash, but about 3 years ago I developed a small, dime-sized (though oval-shaped) puffy skin irritation on my scrotum. It has no discoloration... it is simply puffy, ithcy and unsightly.

The problem pretty much coincided with exposure to poison oak that same year. I went to the dermatologist, who recommended a cream application to be applied to both the poison oak rash as well as this decidedly different problem (he was also rather dismissive about the issue, which increased my embarrassment).

The cream, which at this point I can't remember the name, soothed the itching but did not cure the problem. I have taken a stance of ignoring the affected area over the last three years as it has neither grown in size nor has the condition worsened. Nonetheless, because it is still itchy, and the appearance of it is still puffy, with the affected skin area appearing very dry-looking, I needed to find a solution to this as it is causing me discomfort and embarrassment.

What do you think the problem/solution may be?

Don't know.  Can't be anything serious, though.  Perhaps it's localized eczema that you rub.  If so, hydrocortisone plus leaving the spot alone will take care of it (and no rubbing!)  If not, then perhaps you should seek another opinion.  But this in no way reminds me of anything serious or sexually transmitted.
