Did he take any medications or applied anything on the area before this rash appeared?
This appears like a bruise to me based on the image. It is violaceous in color,flat and there seems to be no redness or discharge in nearby areas. This may also be an abrasion on the skin secondary to an irritation or scratching of the skin. However, abrasions may present with a stinging pain.
The rash is still present even after application of medications. Have this assessed further by a urologist. A penile ultrasound may help determine if superficial blood vessels may be involved or if this is a localized skin condition without involvement of underlying structures.
