Questions about eczema and herpes

Hello,

I've been diagnosed with atopic skin and get eczema here and there once in a while. I also happen to have acne and cold sores, which is why I'm overly paranoid over spreading cold sores on other places of me. I recently developed this rash on my hand and wonder if it's really just a rash or does it in any way look like herpes?

How do I exactly know whether I have eczema or acne or if it is a cold sore breakout? About a year ago I started getting herpes like lesions on a patch of my chin area and it itched and burned like herpes does, but could it have been just eczema? I also sometimes get pusfilled bumps that look like a normal pustule but it appears on the same spot over and over again which is why I get concerned that it might be herpes instead?
