My 13 old brother has rashes around knee and it getting bigger every year.
It is not supper itchey but he iches during his sleep to cause scabs.

Hard to tell from the picture.  All I can see is scabs.  He may just have a habit of picking.  It might be worth a try to keep an ace bandage wrapped around his knee when he goes to sleep.  If that keeps his hands away from it and he clears up, you have your answer.  If not, he should see a skin doctor.
