Redness/burning/itching spreading from mustach to cheeks

I started to develop a burning sensation and redness in the area just above my upper lip (almost like a second mustache) [had no other problems before this, just some average acne]

so eventually after 2 months I went to see a derm and was diagnosed with perioral dermatitis.

I started out for a month on doxy and klaron, and it didn't really help.... they switched to minocyclin and noritate and I've stuck with the mino+noritate for about 8 weeks and have only a small improvement in the burning sensation which waxes and wanes...

The problem is it has slowly been spreading on to my nose, and now it is on my cheeks as well!!! It is also starting to itch A LOT -- My nose has a few patches that look plain white, but then the rest looks red and I have some patches that look really bumpy or scaly... Is it normal for perioral dermatitis to get worse and worse over 3 months of treatment??? and spread to all of the place where I have been applying th creams they gave me??

Also I tried Lamasil AF once when it was mainly just burning in my mustache area and it seemed to help with the burning, told my derm that... but she said no it's bacterial and to trust her... I returned when it spread to my cheeks and she said I should try Xolegel [ketaconozole] around my nose and use the noritate on my cheeks... (I think because I mentioned th AF helped the burning) So for the last 3 weeks it's been Minocyclin, Xolegel around my nose, and noritate on my cheeks... seemed like the klaron was helping or atleast it wasn't getting worse back on doxy+klaron

The Xolegel kind of makes the creases of my nose burn, but it does seem to help with the itching... I also tried klaron on my cheeks/nose for 4 days last week just to see and to be honest it seemed to help with the burning a bit, but maybe not with the redness as much..

So how did this go from some redness and burning above my upper lip to redness, burning and itching on my upper lip + spreading to my cheeks, nose and eyebrows?????
