Spots on penis!

Hello Doc, my name is Johnny and i would like for your opinion on this matter of which ive become very worried about!  I seem to have small bumps on the penis -before it was just underneath and on the scrotum but now i can see it all over. when i look at it carefully I can see small bumps at times i feel it might be clogged pores but now its more visible as some bumps are white like color and sometimes have white heads.  Im really scared because i dont want this to leave scars of my penis!! At first i thought it was nothing because its not painful at all but now im really concerned because i have a partner and i would like to know if this could be something serious! please advise me on what i should do and if there are any over-the-counter creams i can possibly use to treat this issue!!  Thank you so much and hope to hear from you soon.  Johnny

Hi,

For how long have you been having these bumps?

Are these bumps itchy or painful?

Are they dry and do they scale?

Does the skin peel off?

Differentials for this may be a fungal infection ,keratosis pilaris and contact dermatitis. Fungal infection in the groin area may be itchy and they are usually associated with  a red rash with sharp borders. A contact dermatitis may also show a red rash but the demarcation is not as sharp as that of a fungal infection.Also,contact dermatitis is itchy.Another possible differential is keratosis pilaris. This is  a chronic condition that usually affects young adults.It may involve the front of the thighs and buttocks.It presents  as  rough skin due to "plugs" that involve the follicles.

I suggest you have this evaluated by a dermatologist.Topical corticosteroids may be used but at this point the area is very sensitive and in the presence of breaks in the skin or irritation, the use of corticosteroids may not be advisable.Antifungals may be used for fungal infections.

Moisturizing the area with a mild and bland cream , keeping the skin  dry always and avoiding sharing of towels and other personal belongings may help.

Hi and thanks for responding!!  My first guess for this condition i thought that I might have Falliculitis but not quite sure yet!  I have had this bumbs for quite some time now but i thought they would go away eventually but they have now spread all around the penis. They are not itchy nor painful. They are somwhat dry.  They peel off it i peel i peel off the small white heads but dont want to because im afraid this would create scars as some have.  Is there anyway I can get rid of those scars I have one that looks like if i had a pimple its completely sealed but you can stil see where it was and it does not look good.  Do you recommend that it is safe to have sex at this point or should i wait unitl i treat this matter?  Ohh yea- i started using Hydrocortisone Oitment is that safe or should i discontinue using it?

Hi,

You have mentioned of scars and white bumps and aside from the primary differential of a folliculitis, a fungal infection may also be considered here. Fungal infections may present with white skin patches that may peel off and this may also be present for quite sometime. Fungal infections may be itchy especially when affecting the groin area. However, you have noted no itchiness in  your case.

I suggest that you stop using the hydrocortisone cream first until you have been thoroughly assessed by your physician. The penile skin is sensitive and using medications that may be inappropriate for your condition may only further aggravate the condition.
