Two Patches on Each Cheek and a red nose Rash?

I dont know what to do to get rid of this rash or whatever it is? I have this butterfly like rash and have had it for 3 years and cant stand it. It dries when i wash it and then is flaky until i put moisturizer on it but even when i do that it turns a bright red and is like that all day. No cream has ever worked and my skin never reacts to creams even for poison ivy or other minor rashes.It is not acne and i need to know what to do to make it look better or get it off my skin period. Please help

Hello,
Rosacea is one possibility which needs to be ruled out. It is a cutaneous disorder primarily of the central part of the face , such as forehead; cheeks ;nose; and chin. The other possibilities which need evaluation are eczema, Lupur, seborrheic dermatitis and psoriasis skin. In order to confirm the diagnosis please get an evaluation done from a dermatologist.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided.
