Unknown dark spots on foreskin

Hi,

22 year old white male

I have two distinct very light dark colored marks on my foreskin. I have uploaded a picture to show you exactly what I mean:

Are these marks dangerous? I would like to point out that when I was younger I had an issue with phimosis, eventually it subsided and I had no issues at all. Within the last year or 2 I've had some occurences of my foreskin getting dry, red, and irritated. I believe I've slowly been developing Balanitis xerotica obliterans as I have a whitish ring forming that seems to be tighter than the rest of my foreskin.

My main concern is these spots as I will goto my family doctor to discuss options for BXO. Is steroidal creme an option for BXO? or is circumcision the only avenue.

Darker colored “marks” you described are most likely post-inflammatory hyperpigmentation – often seen result of chronic dryness, redness and irritation. They are not dangerous at all.

Balanitis xerotica obliterans (penile Lichen sclerosus) is a chronic, inflammatory disorder. Potent topical and intralesional steroids or topical tacrolimus may be effective in mild cases. More severe penile lichen sclerosus (BXO) requires surgical treatment.
