Weird ring like rash on palms and soles, looks blanched not red

I am putting this here because I have no idea whether this is a rash, a skin condition, or an allergic reaction.  I have been having a problem with a recurrent rash that shows up on the palms of my hands or soles of my feet.  It started last year around November and has come back several times.  It usually lasts about 3 days.  It causes severe itching to the point I can't sleep at night.  I tried hydrocortisone and lotrimin but neither helped at all.  I finally tried benadryl (orally) and it worked.  I have to take it around the clock though.  I am thinking that it could be an allergic reaction to something because the benadryl is helping but I can't figure out what on earth could be causing it.  I have tried finding pictures of a similar rash and can't find anything like it.

I also get pompholx on my hands sometimes (self-diagnosed) but it doesn't look or feel like this.  And, I have been having problems with that since I was a kid.  This is new.  

I am posting pictures.  Has anyone seen a rash like this?  Any ideas what this could be or how to get rid of it?

From the symptoms it can be contact dermatitis, eczema, dermatitis or erythromelalgia. In erythromelalgia, there is severe burning pain (in the small fiber sensory nerves) and skin redness. The attacks are periodic and are commonly triggered by heat, pressure, mild activity, exertion, insomnia and stress.

I suggest you to consult a dermatologist and get all these possibilities evaluated.
