What could be causing this? (lip inflammation - 3 months)

So I've steadily had this inflammation around me lips for around 3 months (likely longer, if anything).   It's constant and does not seem to disappear on some days... it's always there.   So far I've tried:
1. Taking Benedryl (possible allergic reaction?)
2. Hydrocortisone Valerate (as prescribed by a dermatologist... used it for two weeks with absolutely no results)
3.  I fasted for between 2-3 days (probably didn't make a lot of sense, but I was willing to try anything)
4.  Nystatin for thrush (taken orally... didn't really think it would help, but it was worth a shot.   I thought I might have had a yeast infection in my mouth)
5.  Taking a week off (and counting) from using any kind of acne medications... just water and a mild, dermatologist recommended soap.
6.  Following a gluten, soy, and dairy free vegan diet.  --  I try to eat as a vegan for personal reasons, not for this... but I've also tried introducing fish and taking b-vitamin supplements since I heard a b-vitamin deficiency can cause inflammations.

None of these has helped whatsoever.  Anyone know what could be causing this and what else I could try?

There is a picture attached, in case it helps.

They do not hurt or itch.   I do not feel any discomfort around my lips.   This is purely cosmetic.

Hello,
It can be due to perioral dermatitis. Perioral dermatitis is a chronic papulopustular and eczematous facial dermatitis. Treatment consists of oral antibiotics and anti-inflammatory drugs. A mild soap or soap substitute, such as Dove or Cetaphil should be used for washing. Scrubbing should be avoided.

If the symptoms are not controlled by topical antibiotics, then oral antibiotics may be needed. Please consult your dermatologist and discuss this with him.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. For exact diagnosis, you are requested to consult your doctor. I sincerely hope that helps. Take care and please do keep me posted on how you are doing.
