What is this?

Hey Doctor...I asked you about this before...but now I find myself worrying again.  I've had a rash in the crease where my buttocks meets my leg for 6 months now.  It's pinkish and not really raised.  Its just caused bad chafing and been very irritating when I've sat down.  You recommended Hydrocortisone and Lubriderm lotion since you said it's probably just a skin irritation.  So I think its been improving, up until 4 nights ago.  I didnt shower that day, and I did sweat a lot from working out.  I put lubriderm on it a couple times already.  As bedtime came, I put a bunch more on it thinking it would help. In the middle of the night it started itching like crazy.  I looked at it in the morning and I had little red bumps all over that rash, and on my inner thighs, and the back part of my scrotum.  All of these parts had gotten Lubriderm on it.  These areas are really sensitive and itchy.  Todays the fourth day, and it's not as sensitive, but still itches.  I fear that this could be some sort of herpes surfacing. The rash did start a day or two after a sexual encounter without a condom, again that was 6 months ago.  The rash has never come and gone over the months, it has always been consistent.  But these bumps have given me anxiety like you wouldnt believe.  What do you think it is?  Thank you very much for your time, and I look forward to your response.

It is possible the condition is what is known as "Dhobi Itch" (colonial term) - i.e. caused by laundry detergent not being rinsed out thoroughly after laundering of garments, particularly underwear.  Don't use strong laundry power such as Sunlight.

To update a little more, it really doesn't itch much anymore, its more just irritating, and chafes when I walk.  It's still sensitive as well.  They are not blisters or anything, they're just tiny red bumps all around the original rash, on the insides of my thighs, and on the back of my scrotum.

I think it's irritation, as I said before.  Nothing in your description sounds like anything else.  It most certainly does not sound anything like "herpes surfacing."  You need to have a doctor give you a definitive diagnosis, so you can stop worrying about this.  Also, a prescription-strength cortisone cream may help, in addition to the Lubriderm.  Avoid friction in the area when you work out, as much as possible.

Best.
