White Spots and skin discolouration on Penis

White Spots and skin discolouration on Penis

I had sex 6-7 months ago which resulted in white spots on the glans (glands) and skin discolouration on the inside of the foreskin. Also I was leading a very unhygenic life at that time. I showed it to doctors who said that it was fungal infection and treated with miconozole nitrate cream. but wioth out any effect. Later Clobitasol Propinate cream was prescribed along with 10 days course of Doxycycline. But nothining happened. The white spots and skin discoloration refused to go away. I took a second opinion and I was told there os nothing to worry about it was diagnised as Vitligo and then Lucoderma. There is no irritation, no other symptom to suggest a STD. But now white spots have appeared at atleast 2 other places on the body including face, stomach and back.

Hi,

How would you describe these white spots ?

Are they flat or elevated?

Are they scaly or shiny?

Are they itchy or painful?

Do they easily bleed?

Are there instances wherein these white spots ulcerate and drain fluid or pus?

Are there any associated urinary problems such as burning during urination?

Any problems with sexual response like a deformity or pain during an erection?

These are just meant to clarify some points.Do keep us posted with your answers.

Hi there Vannessa,
Thanks for responding. Description of white spots as follows:-

they are flat approximately 1/2 cm to a cm in radius on the glans (glands). they are plain white in colour sometimes appear to be red/ pink.
They are not itchy or painful
They do not bleed at all.
They do not ulcerate or drain fluid or pu.
No problems in urination except in rare cases some amount of irritaion in the urether.
No deformity in errection. but loss of libido due to feeling that this may be transmissible.
Indisde of foreskin regaining its pigmentation and healed approximately 40 %.
Wonder when it all heals.
Urine tests all clear. No special STD related test done as yet. All on judgement of doctors consulted.
Thanks

Hi,

Thanks for your answers.

Flat white and asymptomatic patches in the genitals that are also noted in other parts of the body may have vitiligo as a primary differential.Your skin condition did not respond to the antifungal medications. Vitiligo needs close management by your physician. It is a chronic disorder of the skin pigment. Corticosteroid therapy is the mainstay of therapy. If your case is vitiligo, then this is of the localized type.Topical corticosteroids may be able to help. Discuss this with your physician.A second opinion may be necessary.

A skin biopsy of similar lesions on your back may be able to help with the diagnosis.

Hi,

Are you male or female ?

Based on what you have posted , this sounds  like a fungal infection or a psoriatic lesion.

“Psoriasis is the most common inflammatory reaction affecting the genitalia. It may appear in 2 forms. Patients may develop bright red well-defined inguinal plaques known as inverse psoriasis. The scale so apparent in other parts of the body is not seen. No central clearing often seen in tinea is present. The plaque appears homogeneously erythematous. Similar lesions may be affect the axilla or the popliteal fossa. Unlike psoriasis elsewhere, inverse psoriasis may be itchy. Often, no past history of psoriasis is present.”

Erythematous means reddish  and popliteal fossa refers to the area behind the knee.

Do you have any similar rash in other parts of the body?

Any discharge from the genitals?

As this is related to your case, may I ask if you are sexually active ?

I suggest that you have this assessed by your physician. I can not give you suggestions for OTC medications since certain products when given inappropriately may only aggravate the condition.

Wash the area with mild and bland cleansers and keep the area clean at all times. Avoid using tight underpants and use only underpants that are of light material. Avoid sexual activity until this has been fully evaluated by your doctor. Also, refrain from sharing towels as fungal infections may be contagious.

You may refer to the above url for more information on genital skin conditions.

Do keep us posted with regards to your progress.

Dear Vanessa,

I was prescribed Ketaconozole 200 one tablet daily. The white spots on the glans (glands) seem to have disappeared to be rplaced with reddness and itching sensation. also the discoloration beneath the foreskin has become sensetive and itchy and redness is there. I wonder where does this lead to ? any suggestions from your side.

regards
