breakouts under jaw

hi. i have been getting breakouts under my ears and around my jaw for three years now. they are very sore and sometimes make my face hurt around the area. the spots usually come to a head and are often rock hard. my skin is very dry generaly and very rarely has any blemishes and i have never had any broblems before this. i do use very heavy prescription creams which i thought could be causing this, but i have been using the same stuff all my life and never had this problem. so was wondering what this could be or what i could try to help. thank you
