enlarged and itchy testicles

My son is in Japan on holiday. He thinks that he is having an allergic reaction that enlarged his testicles. He says that they are about 3 times larger than usual and it feels like the two testicles have become one giant testicle that is the size of an orange. The skin is harder than usual. The area is itchy.
He went to a dermatologist in Japan and was prescribed an ointment. After the first 5 hours there is no change. We have no way to know if this doctor is good or if we need to see another doctor. The main worry is for his health and to prevent sterility.
