This may be a fungal infection. You may use an over the counter antifungal cream over the area and see if this responds. I do suggest however that you inform your primary doctor about this. I would still suggest an initial consult in case the medication is inappropriate. Another differential for this is a plantar wart, a callus or a psoriatic lesion. You have noted this to be red, scaly and presents with bleeding .If this is the case, do not apply anything on the area just yet if there is bleeding or an open wound. Allow the area to heal first. We do not want to further aggravate the condition. Fungal infection is a likely diagnosis. You may apply the antifungal cream on intact skin for at least a week. If there is no response, consult your dermatologist.
There is always a red circle about the size of a nickle right in the middle of the bottom of my foot. It does not hurt, but is very itchy. When I scratch it, it becomes scaley and flakey and will sometimes start to bleed. Is there any over the counter medication you would recommend to treat this?

Differentials for this will be an irritant contact dermatitis, plantar wart, fungal infections or a contact dermatitis. A plantar wart is skin colored and may present with a thick roughened skin on its surface.You may find dark spots over the area also. Skin conditions associated with scaling may suggest a fungal infection or a dermatitis. These two conditions usually present with itchiness.
I suggest you have this assessed by your dematologist. You may need topical medications for this. A microscopic evaluation of the skin scraping in the area may help differentiate between the mentioned differentials.
Is there any redness in the area?
Any pain or blisters?
