glans bump

Dr Rockoff

I'm a 40 year old married man. My wife and I are monogomous for the last 16 years. She has genital herpes. I have never had an oral or genital breakout ever.

About 5 weeks ago I noticed a bump (much like a mosquito bite) on the under rt side of my glans (glands) about 1cm from the tip. At the same time I had pain and mild swelling on the right side inguinal lymph area and a similar bite like bump. This spot was different in that there was an intense itch. We had been swimming quite a bit and I just thought a bug may have gotten into my suit as I had left it outside to dry. That passed after about a week. The bump on my glans (glands) never filled wit fluid nor did it burn or itch. I showed it to my wife and she said it did not appear to be herpes as she experiences it. It did appear to have a spot or hole in the middle though it never oozed or leaked. It very slowly has shrunk and is gone only I can still tell where it was as there appears to be a small whit spot or fleck maybe less than 1 mm. There no longer appears to be a core. It is no longer raised or irritated.

Additional considerations
My wife has not had a breakout in at least 4 months prior to my experience nor has she had one since.
prior to the bump It had been at least 10 days since the last time we had intercourse (period etc)
Two nights before I noticed the bump we had a fairly aggressive rubbing mastubation type coupling no insertion.
We do not engage in anal sex but that night we played and i had rubbed myself on her anus no insertion just playing
Finally, we have had our family, i have a vasectomy and throughout our relationship we never practice safe sex, only abstinence when she has breakouts.

Questions:
1- Could i finally have aquired her herpes
2- Could I have given myself some type of infection by playing near her anus
3- could it have been an insect bite
4- What are your thoughts what do you recommend. Do you think it is ok to have intercourse as we have been abstaining.

Welcome to the Dermatology Expert Forum!  I carefully read all four of your posts and, based on information you provided, answers are:

1. The way to find out if it is genital herpes or not (especially when asymptomatic, or with mild, atypical clinical manifestations), is testing.

2. Urogenital contamination with microorganisms from anorectal region of a sexual partner is significantly more probable during anal sexual intercourse than just rubbing. In addition, spontaneous resolution of such urogenital infection would be highly unlikely.

3. It is possible. Insect bites have that redness plus mild swelling plus itching aspect, and many of them tend to heal even without treatment (a bit slower though).

4. Since clinical findings were not specific enough, list of differential diagnoses would be long. Therefore, based on medical history provided, HSV-2 should be excluded/confirmed first.

5. Sweating does promote fungal/yeast infection, so is wearing tight underwear made of synthetic materials. However, two small lesions (you stated 1-2mm in diameter) that subsided spontaneously (without any changes in your lifestyle or use of therapy) do not point towards jock itch you were worried about.

Additional consideration.

I'm a runner. I may do 5-7 miles a day. Clearly I sweat a lot. Occasionally It may be over a few hours befor I can shower. I've never had jock itch but thought maybe it was caused by sweat.

Thanks again

Dr,

I was reading back trying to give you as much detail as possible. I just wanted to clarify:

The bump on my glans (glands) was located (looking down) on the glans (glands), on the underside and to the right of the opening. But definately on the glans (glands).

When i said like a mosquito bite, that was initially what it looked like. Then, as it healed it resembled a small pimple like a white head but not blistery. It did seem to take almost 5 weeks to go away though it was mostly gone after about two weeks. Then over the next couple of weeks it just appeared as a tiny white dot. Not raised in a discernable way.

I've exhauseted all my search areas and just can't find anything that sounds close. Tysons Gland sounded close but it is not on my frenulum. It is on the glans (glands). Sebaceous cystsa and pearly papules sounded close but this was not on my corona and there are not multiple spots.

Finally, if I went to a dr today I'm not sure they would be able to discern what I think I see. I waited too long to go.

Thanks Again,

I'm glad you havn't replied yet as I've continued my investigation. If you read all the way down to this I'd wonder what you'd think of this.

Given there was no associated pain, burning, blistering etc. There was never anything even close to a cm, more like 1-2mm at best. My wife nor I have HPV. It was never an ulcer and given most definitions most resembled a pimple or papule. It seems that I experienced a clogged or infected pore or sebaceous gland. If that is the case do they take as long of a time to resolve (3-6 weeks)? Like I initially wrote, for the most part, the irritaion lasted about 2 weeks but the whole episode took until now. Again, at this point it appears to completely be gone but upon very close inspection there appears to be the smallest white dot. From reading it seems that may be a scar or sebaceous fluid. Well, thanks. My wife and I were curious of your reply. Hopefully you will reply soon. Again, Thankyou
