hair fall, itchyness,white oval thing attached on the root of the hair

I am now 23 and have been suffering from hairloss since I was 14-15. I reffered to some docters in past but they didn't show interest in my case some of them never examined my hair and some of them just blamed heridity.
What i personally observe is that I have small white oval type of thing attached on the root of my hair or the skin. I literally can feel this  thing in my head even after  washing my hair.If i don't wash my hair even for 1 day my scalp gets greasy and i even get greasy (brownish type) layer on my scalp(like some kind of oily dandruff & with a bad odor) . Then it becomes worse as it itches too .For that reason I use/must to wash my hair daily.In addition to that  i also can literally feel  some elevation or bumps(not so many).
Besides hairfall i am also suffering from premature hair which i am dealing with the use of coloring.But I am more concerned with controlling of hairfall.
Well this is my problem and i guess this particular problem is supporting the hairloss as i loose my hair everytime i comb or wash my hair. I don't know which disease to refer ; scalp or seborrehoea?

I know my query is getting longer and longer but I'd really appreciate if you please read it once and suggest me as following because this hairfall thing is really eating me from inside.

For the hair fall, causes could be due to stress, fungal infections, alopecia aereata, hormonal imbalance (overproduction of dihydrotestosterone and thyroid hormones irregularities),malnutrition, telogen effluvium and sebum plugs on skin.

I would suggest you to consult a dermatologist and get these possibilities evaluated. It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided.
