can dyazide, prilosec, or phentermine cause hair loss?

Hydrochlorothiazide(dyazide),  a 'water pill,' is used to treat high blood pressure and fluid retention caused by various conditions, including heart disease.
Hair loss is documented as a side effect of this drug.
