The craters are undoubtedly atrophy (skin-thinning) from the cortisone injections. This is temporary and likely to fill in over the next 6 months. I can't see any connection between your tiredness and either the hair loss or the craters--little cortisone is absorbed.
Whether you need steroid shots is another question. They are used for alopecia areata, which produces smooth, round bald

Mongolian blue spots. This doesn't sound like what you have. I think you need a more definitive diagnosis. Perhaps another opinion with a specialist in hair disorders at a medical center might be worth considering and discussing with your current dermatologist.
