Hello,
I cannot confirm anything without examination but it can be contact dermatitis, allergic reaction or epididymitis. Pain from epididymitis (an infection of the testicle) and other causes usually sets in gradually. Early on, pain and rash due to epididymitis is often just in the area of the epididymis itself.

My sincere advice would be to consult a dermatologist/ urologist and get it evaluated.

Hope it helps. Take care and please do keep me posted on how you are doing or if you have any additional doubts. Kind regards.

Hello,
Unfortunately as per the rules and regulations of the forum we cannot divulge any personal information. Any correspondence has to be done through this forum. From the symptoms that you have mentioned, the chances of epidiymitis, testicular torsion or hernias leading to testicular swelling are more. There is no relation between appendix and testicle. So appendicitis cannot cause Testicular swelling. Please consult a urologist and get the diagnosis confirmed. Scrotal ultrasound may be needed.
I hope it helps. Take care and regards.
