and it is always wet. Small part of skin under the foreskin has become red and also there is pain when I touch it.
On the head itching is no more there but a part of head has become dark colors.
For last 4-5 days I started using "Quadriderm" cream which contains Clotrimozole IP.
Please suggest me what to do.

You could be having an inflammation of the penis/foreskin. It is usually seen in uncircumcised men due to poor hygiene. Other causes are dermatitis, allergies, infection such as candida, gonorrhea etc, eczema. Treatment depends upon the cause of the rash, antibacterial or antifungal .ointments can be prescribed by the doctor. It is important to keep the area clean and also to avoid strong soaps or chemicals which can cause a skin reaction.

You need to wash the areas several times with fresh water. Do not use any cosmetic products at the sites.
Apply calamine lotion at the site of the lesions and see if it helps. You could take some oral antihistamine medications like cetrizine or loratadine. You need to maintain a good personal hygiene .

For mild-moderate eczema a weak steroid may be used (e.g. hydrocortisone or desonide), whilst more severe cases require a higher-potency steroid (e.g. clobetasol propionate, fluocinonide).
Eczema can be exacerbated by dryness of the skin. Moisturizing is one of the most important self-care treatments for sufferers of eczema. Keeping the affected area moistened can promote skin healing and relief of symptoms.
