pimple on child genital

Three months ago my 3 years old boy appeared with this kind of pimple (no white head) around his eyes and penis, He doesn't itch or cry when he goes pi; I kept calling his doc, that told me it could be rash from soap or something else.. I changed the detergent and soap and no more bubble baths, but it didn't go away, the pimples now are spreading and getting bigger with white heads, I am so frusted calling office and just talking over the phone; and apply this .. for 7 days then call me again; Would you suggest something different?
