rash on joints

I have been getting a rash on my wrists and elbows occasionally for about 5 years. Usually at the beginning of each episode, I have pain in one joint (ie. foot, wrist) but not always in the same joint and not bilaterally. This time the rash started as usual, but spread up and down the entire length of my arm. I also have a rash on the finger and toe joints, all around my neck and now spreading (after 2 weeks) to my back. It itches badly. It is NOT scabies or shingles. (I am a nurse and very familiar with both.) So far I have been told that it was psoriasis, then eczema, then contact dermatitis. None of the steroids, etc have worked. I should also mention that I have red scaly patches around and on my scalp frequently and sometimes in my eyebrows. Usually coal tar shampoo keeps it in check. Heat seems to worsen both rashes. Any ideas? Have seen GP and 2 Dermatologists.
