red, hot rash on my hands

I am on antibiotics and was in the sun last Sunday and now I have a red rash on my hands that hurts and is hot. I used sunscreen but I was swimming and it must have come off. It is really painful not to mention unslighty. I have been using over the counter cortisone cream but it doesn't seem to help.

What you describe seems like sweat dermatitis or heat rash.
You should try to keep the area dry and clean. Try some powder to absorb the sweat.
If the symptoms are still persistent, consult your dermatologist and get a proper clinical evaluation done.
Meanwhile, avoid sun exposure and use a medicated sunscreen.
