red itchy bumps / scabbing

i don't know what this might be, but it all started when i was about 11 -12, i got out of the shower one time and these bumps the center of my chest ( inbetween the two pecktorials ) were itching / had red bumps. i ignored it because i didn't think it was a big deal, but everytime i showered it would itch like crazy. i just kept on ignoring it. now i'm 22 and theres 3 spots, one in the same place, another just above it and a smaller spot just above that one. at night about 2 years ago i used to have these break outs, that i would scratch really bad. i then started to wash with soap and warm water, and the itching would go away. but the next day they would scab over. and maybe a week later or few days later it would come back again... they all have the same characteristics, each one has a surronding area of brownish / yellowish skin, and in the center of that is red bumps that spread out, ( its doesn't itch much or at all really anymore ) but i'm concerned that i still have this problem, i still see red bumps, i have used bacitracin ointment, and when i apply it, it starts to itch again... how do i get rid of this completely and get health new skin once again?
