redness itching, penis area

about every two months i get like a rash, its like redness and itching on my sack and a little up the pipe. If not treated it can go on for quite some time. I use dr rx econazole nitrate cream, even with that it takes at least three to four days to go away. What i don't understand is why does it keep returning. Is it something that im doing?? How can I avoid it. also I have no insurance right now.
