spots on arms upper chest and upper back

I ve had this as long as I can remeber.  Starts out like a little bump thats red.  Kinda itchy, its flacking.  Then its like it grow into a nickle size white spot and I have no tan in this spot.  itchy and dry feeling.  Ive tried lamisal, dandruff shampoo, foot fungus spray. Nothing,  Also worse in winter time very bright red and itchy when i get out of shower.
