thumb infection

have quite bad infection on left thumb (top - pad) and in nail. The skin is sore to touch and constantly getting cuts and peeling. The thumb nail has under it a white mark which spread over last 2 months so that it now takes up about half the nail bed from the top end(cutting end) - my GP took a nail sample for analysis and it has come back neg for fungal infection of any kind - I have been told it is a 'Chiropodist issue' which leaves me baffled. Thumb so sore at times that I cannot bear any touch on it...  - I was put on a course of antibiotics and fungal cream when it first appeared, but no use.  any ideas what is wrong and how to cure?
