DM ,HTN, Dislipidemia

dear ,
my lab report:
plasma glucose(random) 384.0 mg/dl
Lipid profile (fasting)
Serum Cholestrol              213 mg/dl
Serum HDL Cholesterol :   32 mg/dl
Serum LDL Cholestrol    :  138 mg/dl
Serum Triglyceride         :  490 mg/dl
SGPT(ALT)                    : 69 U/l
HbA1c                           : 12.2 %

pls suggest me about my condition , what medication should be taken ?

You need to see your doctor urgently.
This results show you have out of control diabetes, with dangerously high blood sugars.
  
You may need oral meds and or insulin (nromal HBa1c should be < 5.0).

You should be also following a low carb diet, and exercising daily.

Do not neglect to go back to your Dr.  You need medical as well as lifestyle treatment for this.

Your blood sugar is too high and your A1c is too high!  You need to go on a strict diet and ask your doctor to put you on medications.  Your cholesterol is better than mine was when I originally was diagnosed with diabetes.  My blood sugar fasting was 134 - my A1c was: 6.9 and my total cholesterol was 298.  I took a blood test 2 months after going on a diet and my cholesterol went down to 234 but my A1c went up to 7.3; now it is 6.2 (A1c).  You really need to consult with your doctor and you may also need to lose some weight.  I hope you get a dietician to help you after you see your doctor, please keep me updated. those BG numbers are WAY to high.

glucophage and or insulin AND a low carbohydrate diet (no rice, no pasta,no potato, limited fruit...)

We do know ALL carbohydrates raise BG  ALL carbohydrates
