Diabetes Query about Insulin

Last year I was diagnosed with diabetes with fasting 330 and PP 569.

I consulted a doctor and he asked me to go for following tests and the readings were:

C-Peptide, Serum (CLIA)                            2.91 ng/mL

25 - Hydroxy Vitamin D3 (ECLIA)               28.75 nmol/L

HbA1c                                                         7.70

I was advised to take Cetapin XR 500 mg. It was helping and the levels came down. From last 3 - 4 months, the levels start increasing.

I consulted the doctor again and he advised me to take Glimy-M (2 mg) and for the tests again.

Now, I went for following tests and respective readings are:

C-Peptide, Serum                         1.09 ng/mL

Vitamin D, 25 Hydroxy (EIA)         43.72 nmol/L

HbA1c                                           11.00

Based on these reading the doctor has asked me to go for insulin and following medicines.

Insulin Insugen (30/70)               16 Before Breakfast              14 Before Dinner

Calcinol Sachet                           Once in a week with milk for next 3 months

Tablet Galvus (50)                      1 after breakfast                    1 after dinner

I am confused and consulted another doctor and he asvised me to go for GAD-65 test.

Need your help. Kindly let me know, what to do.

Should I start the insulin? Please help.

To answer your question, yes, I beleive you should go on insulin as instructed by your doctor.

The answer above also very good.

Please work closely with your doctor.

First thing first, what are you doing to control and manage your diabetes?
* Are you following a diabetic diet?
* Is your body weight normal for your height?
* Is your BMI index in the normal range?
* Do you perform daily 30 minute physical exercise/s?
You must adhere to all the above or your glucose will get out of control. Your elevated A1c [HbA1c] indicates you are not practicing one or more of the above, the next step being put on insulin.

Tests briefly explained:
C-Peptide tests the amount of natural insulin you still make
GAD-65 is a type of antibodies our bodies produce when our immune system is attacking our Beta insulin producing cells.

If your GAD-65 results are positive read up on LADA [Latent Autoimmune Disease in Adults]. Usually C-Peptide and GAD-65 are ordered if LADA is suspected. Since every lab, and especially a lab in India, differ in their test result ranges we cannot comment further on your posted results. Hopefully your lab copy provides low - high ranges which you can discuss with your doctor/s. Good luck.
