With your father's blood sugars being so high metformin alone will not control his blood sugar.  He definitely needs to be taking insulin as well, and preferably using both a long acting insulin (injected 1 or 2/day) and a short acting insulin before meals to cover his food and also to make corrections, if required.

Yes. Hyperglycemia and high diabetes definately causes fatique. Once the blood sugar is under control, the fatique level should show improvement. However, Diabetic patients taking metformin are prone to have B12 deficiency. So B12 Supplementation helps in fatique.

What may happen if no insulin is used but only metformin tablets are used

Yes, it is so serious.  He can die.

Please take him to see his doctor, and also make sure that his medication is adjusted to get his blood sugars down to a more normal range.

You are not concerned that he is sleeping so much?  If one sleeps so much caused by a medical condition that is not controlled one may just die in one's sleep.

hello,

these numbers are dangerously high.  He needs to have his medications adjusted (insulin) and carefully managed to bring down his blood sugar.  he is in danger of diabeteic ketoacidosis.  This can be life threatening.  Please take him to see emergency room doctor if he cannot get in with his regular doctor very soon.

If you could state your numbers that may help. If you are regularly in the double digits or over 20 mmol/L it's definitely not good. If your breath has a strong fruity smell along with extreme tiredness please go to hospital and tell them your numbers and symptoms.

the concern is "high numbers"  High numbers will lead to fatigue , neropathy, loss of circulation, leading to gang green and loss of toes and feet/leg , retinopathy leading to blindness.

BG control is essential to reducing the complications of diabetes.

Carbohydrates raise BG
metforman, insulin and other drugs lower BG
exercise lowers BG

you have to find the balance that works for the patient
