Confused about H. Pylori - Please help!

I am a 36 year old female who began to have neurological & stomach problems six months ago. No previous health issues. Neurological problems consist of headaches, numbness, and occasional aches & pains.  Stomach problems consist of chronic stomach aches in upper area and loss of appetite and weight (15 pounds in 4 months).  Tested low positive for H. Pylori (reading of 114) through a blood test.  All other blood tests were fine.

Endoscopy this month indicated mild hiatal hernia and gastritis.  Biopsy revealed no H. Pylori.

Gastro doctor prescribed Nexium, which I started a week ago.  Symptoms still exist.  

Doctor did not prescribe treatment for H. Pylori since biopsy was negative.  I asked him why the blood test was positive if biopsy was negative and he explained that a strong regimen of penicillian I was on back in September (for treatment of a high ASO blood test) most likely destroyed the H. Pylori.  Is this possible?

Also, I have researched H. Pylori on the internet and discovered that long term, untreated H. Pylori can cause neurological problems and possibly a stroke.  Is this also possible?

I was also told that a mild hiatal hernia is common and benign and would not be causing my symptoms and would not require treatment.  Is this true?

Any advice and or information would be very much appreciated.  Thank you so much!

I turned 40 years old in JAN 2004 and during the month of DEC was when I developed a burning directly behind my breastbone. It helped a bit when I ate but was present 24 hours a day until FEB when I saw the gastroenterologist on THURSDAY (FEB 5th). Blood tests revealed a huge infection of H PYLORI. DOC gave me PREV PAC which tastes absolutely hideous (like metal) but I am hoping it will cure it. Right now, I feel like a major battle is being fought in my stomach but my pain flows up to my mouth. I feel like there is a pull in the back of my throat , continuing approx 2-3 inches below into the esophagus (but it's just my impression). Perhaps the H P is causing the entire problem. All I can say is I drank a lot in December but nothing out of control. My diet is pretty good. I am 5'5" and weigh 130 (since I stopped working out 5 years a go I gained 10 lbs) and I wonder if that started my downward spiral as I NEVER had any problems in my 20's and 30's. Eat a fair amount of veggies and fresh sruits, drink a lot of water...do not get why I got this. I read on the NET that HP can cause gastric cancer and people with it are far more predisposed to getting stomach cancer. Not good to hear -- but has anyone here been totally cured after taking PREV PAC (which I'm on) without any recurrence? Thank you for commenting. I feel terrible physically and emotionally. (I'm also planning on starting daily workouts again but think I should wait til this aubsides). THANKS. PS: Does anyone have a really fine doctor in NYC? Not sure about my current gastro..

14 days of prev pac treatment will upset her flora in her body,too much bacteria wiped out and yeast will grow rapidly.
she may have a case of yeast infection.

A few weeks on from having had triple therapy and our daughter is still belching - is this common/normal after this kind of treatment?  She is now on omeperazole 20mg daily.  We also went to a health food shop and are trying her with manuka honey and a multi vitamin with probiotic nutrients.

I WAS HAVING SEVERE ABDOMINAL PAINS.i COULD NOT KEEP MY FOOD DOWN AND WAS LOOSING WIEGHT RAPIDLY. After a misdiagnosis of a kidney stone I went to a Doctor whom was also a old friend.He said because the pain was abdominal but did not go to the groin he checked my back.Directley across from my belly botton my back was out of place.I had no idea my back hurt till he pressed on it. I noticed many people writing to you have abdominal pains.Try a stretching execises such as putting your foot on a chair with your leg straight.If you feel lots of cracking in your back you might need a chiropracter or body alignment/stretch program.

Hi Redwen,

Sounds like you might also want to have another h pylori test [the UREA breath test will show if you have an active infection].  Read my post on, I believe, 1/6/04 entitled "Stomach Issues."

Hi Gez,

I just finished h pylori treatment last week and have had stomach bloat, indigestion, etc. since treatment ended as well.  I did find someone else on my heartburn board who said that even though further testing indicated she had gotten rid of h pylori, she continued to have symptoms for quite some time afterward.  So maybe it's not so uncommon.

However, it's best to have your daughter re-tested again as soon as the doctor thinks it's practical to make sure the infection is gone.

Bad night last night - daughter awake - belching, feeling hot, etc.  Does anyone know if this is a side effect of erradicating treatment for h pylori?  Finished triple therapy over a week ago and now on omeperazole 20 mg daily.  Not much advice given from doctors.  Due to see doctor again at end of month.

Wow I too am suffering from the constant stomache pain.  I had a blood test done for H.pylori and it came back positive.  Dr. treated me with prev-pac.  I was amazed at how good I felt for the next 2 months.  However, within 3 months I was back to the pain.  I also had gained almost 20 lbs.  Some of the pain feels like extreme hunger pains.  And eating made me feel better.(but not in the mirror!) With-in the 2 wk therapy prev-pac I LOST 15 lbs.  Now the pain is there constantly.  I saw a "specialist" he said he thinks I have IBS.  He is treating me with Pamalor.  The info I find is this drug is used for headaches.  Anyone else useing this?  It doesn't seem to do much.  He wants to do an endoscopy but I am $ maxed out.  So research! I wish health to all of you. Redwen

Thanks for all of your replies - I am just very confused by all this.  So, if my blood test was positive, and my biopsy was negative, I should request a breath test to see if I am still infected by H. Pylori?

I was not on any meds during my blood test or biopsy so that did not effect the results.

My questions are:  If I am infected by H. Pylori, would it cause the chronic, constant ache in my middle, upper abdominal (always the same spot)?  I do not have ulcers.  Or, would my hiatal hernia be causing this symptoms?

Also, I researched H. Pylori and found that it can cause neurological symptoms (which I have) and even stroke.  Is this true and why does this happen?  I'm really worried about this.

I really appreciate everyone's help.  Thank you so much.

  A breath test will accurately tell you if you are presently infected with hp or not. That is the test given to see if eradication is successful-- as mentioned above -- a blood test is no good-- it will show positive for years regardless.-- but a breath test is accurate

  H pylori can be-- and is missed on endoscopy frequently if the wrong spot is biopsied.  It is also now reccomended by many GIs that you be off of PPIS 2 weeks prior to endoscopy-- as this can also cause a false negative result when biopsies are lab tested.

   If you have ulcer symptoms you definately should be treated -- although hp rarely causes symptoms-- it normally takes years for hp gastritis to cause problems- so treatment may not necessarily alleiviate symptoms.

  The most important reason to treat it if you test positive-- is-- it can eventually lead to stomach cancer. -- I have gone through 3 or 4 treatments myself and still test positive on a breath test -- but have no symptoms- so it's not something i'm terribly concerned or care about.-- in the near future anyway

The Nexium treated what Nexium is suppose to treat (stomach inflammation...in your stomach proper and duodenum, the "acute burning" to quote which has improved).  The gas/pressure sensation in the lower abdomen after eating is most bowel in origin and the Nexium will not treat this.  The discription you give is very consistant with an underlying irritable bowel, but it should be investigated further.  I would stay on the Nexium, and see your specialist again.  Keep an open mind about the fact that there may be more than one thing going on...OR that the symtoms may all be consistant with dypepsia/irritable bowel picture and may need treated along the same lines.

Hi there. I'm a 31 year old male and I am experiencing nearly  the same simptoms like you.
Headaches are not very often but 2 months ago it all started with burning feeling in the upper part of stomach, left and then right side. The burning sensation extended few days later to whole my stomach. I tryied usual medication available in Superdrug shops without any good efect.
After 1 month of struggling with my problem I decided to see a private specialist doctor. I paid a lot of money and he performed an endoscopy. The results are: moderate hiatus hernia with gastritis, and an iflamated stomach. Good duoden and no ulcers. Biopsy test for H.pylori bacteria was NEGATIVE. Also, ultrasound-scanning reavealed NO problems with my GALL BLADDER.
The specialist said he reckons my inflamated stomach may be the result of the stress at my work-place (i cannot accept this explanation!). Hi prescribed me Nexium for 1 month which I have done it... but without any good. My symptoms stil persist. I refuse to believe this is caused by stress, and I'm upset that he never performed any blood tests. I may see him again shortly.
Also he said that my moderate hiatus hernia cannot be treated succesfully. It is an unusually disease for a young person like me and I cannot do anything about that. It will help me if I sleep gradiently (with the head slightly up) to prevent the content of the stomach coming back to my throat while I am asleep, and to lose some weight. He said that the hiatus hernia will get eventually worse with the age, and after years, if the worse comes it exists some medication for that and even a "small" surgery which can be performed in connection with that.
I lost indeed about 12 pounds in the last month, but I don't know if it is the result of my diet or the result of my disease.
I'm desperate to find an answer for my problem and I'll be watching closely this forum. It may help me a lot.

The "blood test" for h-pyrlori will remain positive years after you actually have it.  So, in your case where you were treated with a large dose and duration of antibiotics a few months ago and now your stomach biopsy (the BEST way looking for a h-pylori infection) is negative, I agree with your doctor that treatment of the h-pylori with antibiotics is unneccessary.

If the endoscopy did not show the reason why you are having weight loss and upper abdominal pain, a CT scan of the abdomen (looking at the Pancreas) and perhaps a gallbladder work-up (ultrasound and maybe even a functional test) can be the next steps.  Your doctor may also consider raising your dose of Nexium to twice day dosing for a few weeks to see if this is helpful.

Prilosec, nexum, protonix, prevacid are all PPI's, same family. They reduce stomach acid production by 80-95% almost. True, some work better than others. Protonix is my least favorite, looks like prevacid seems to work better for me. Nexium I hear is actually the best of all but this is not to say it'll work the same for everyone.

Make sure she is tested for H. Pylori, but not thru blood. I had H. Pylori, blood test came back negative and my biopsy came back positive very shortly after. If you are on PPI, it could mess up the blood results. My advice, like I myself, did is for your daughter to bite the bullet and get an endosocopy done. They can view the esphoagus, the stomach and the deudoneum. They will biopsy all areas and examine for H. Pylori. Barium swallow and all this other tests they do are useless if you ask me. They need to get in there and check it out. If there is ulceration, inflammation, or infection by H. Pylori, they can all be ruled out.  

Your daughter has to learn to speak up or you should go along with her and ask the right questions. A complete blood work needs to be performed on here to rule out for gallstones and other things. Request copy of all the results & keep them all in a medical file at home. You have the right to ask these things. THru my experience, I have learned to be my own doctor along with the doctors themselves. I hate being told it is in my head as that was what I was told by one of my GIs. I feel for her like I feel for myself. She will be okay. Keep working with the doctors. Again, endoscopy is the way to go! Best of luck. I was told I have H. Pylori which gave me an sever onset of GERD symptoms, finished treatment about a week ago. I'm on preacid now. I feel better but I have a chronic sore throat with its causes unruled yet. I'm on PPI so it can't be acid is what I tell myself but who knows. Good luck w/ your daughter. I will check with you some other day and see how you've made out.

Hi, all...reading this thread of comments is very interesting because so much of it sounds like what my daughter is experiencing.  My question is: what is the difference between Nexium and Prevacid?  Prevacid is what she was prescribed, and she can't really tell if it's helping.  Should she request to be switched to Nexium?  

Her problem also began with an acute pain in the gallbladder region, along with vomiting every time she ate and feeling nauseous every time she rode in a car.  The pain moved and changed to a more generalized sensitivity throughout her entire stomach area.  Eating made her feel ill, but not eating made her feel worse...she would get light-headed and nearly pass out.  Some days she feels okay, but other days she has a hard time with nausea and light-headedness.  She has not complained of diarrhea (sp?) so there has been no suspiscion of IBS, it seems to be entirely a digestive-tract situation.

I know that she had several blood tests done, so I would have to assume they would have tested her for this H-Pylori.  But then again, I've learned never to assume anything when it comes to medicine and doctors...

She is a 21-year-old and doesn't speak up for herself as I would during doctor visits...how can I pry into this without crossing any lines?  I would love to be able to see her blood test and scan results and speak to her physician myself, but don't know if that would be allowed...any suggestions?  She doesn't seem to want to push the issue, but I'm like a hound-dog about things like this...I pursue it until I get a difinitive answer...am I just a nosy mom or am I on the right track here?

Any comments would be appreciated.

Hi again,

I think I am "finally" starting to feel a little better -- almost two weeks after hp treatment.  However, I still get some stomach pains & indigestion here & there and do still have mild heartburn.

I hope your daughter is well soon.  If not, it would be best to encourage the new GI doc for a follow-up hp breath test.  I believe you need to wait one week after the last antibiotics were taken and that she needs to be off all ppi drugs for 1-2 weeks before the breath test.

Sorry she's suffering like this.  Hope she gets well soon.

Thanks for your comment.  We contacted our GP for advice but are finding him very unsupportive.  He wants to send our daughter for counselleing as he thinks it is in her head even though the bioposies show she has mild gastritis and h pylori type organism.  We have another appointment with another GP at the end of the month.  She says she has never felt so bad in her life.  How long have you been experiencing this?  Gez
