Lower abdominal and back pain

I'm a 35 year old female. I have been having a problem with lower right abdominal pain, that sometimes radiates to the lower right back, a lot of gas and bloating. I have been seen different doctors for this problem and I have had several tests performed, including blood tests (normal), urinalysis (normal), stool (normal), abdominal ultrasound (normal). Two years ago I had laparoscopic surgery where some ovarian cysts were removed. After the surgery, I still had the same pain. A gastroenterologist performed an upper endoscopy, where he didn't find anything serious, only a hiatal hernia that according to him, with some diet could be control. A year has passed, I'm still having pain, and a week ago, after breakfast, I noticed I had a rash on the abdomen (no itching at all). I had cereal and natural orange juice (I have allways had the same breakfast for ages). Well, the next day the rash was gone. I tried variations of the same breakfast for several days and I came to the conclusion it was defenitely the orange juice. How ca I develop orange juice intolerance of all the sudden? The pain is still there, and also I have left shoulder pain, sometimes I itch on the legs. What could all this symptoms be related to? Thanks!

Have you tried the low acid type of Orange juice?
