*Serious* Stomach issues Loss of weight No appetite black tarry bowel movements.

I am a 25 year old male. My past medical history is pretty healthy with no major issues. I used to be able to eat alot and also drink alcohol with no issues.
For the last few months (6months or so) I notice i am not as hungry as normal or eat as much i never thought much of it, but within the last 3-4 weeks I have very bad symptoms that are escalating and getting worse each day. It started out as me not being able to finish dinner due to feeling full quickly, then i notice i dont have large bowel movements like i used to.
Within the last 2 weeks I notice my weight has went from 154lbs (5/26/08)  to 143lbs (6/15/08)

6/1/08 8pm - went to the emergency room for what felt like upper abdominal pressure and overall abdominal pressure, they did xray scan that came up normal and did blood test - normal than gave me an antacid cocktail and told me it was just heartburn so i went home even though i still did feel dscomfort,

6/1/08  --- 6/1/08 for the next couple of days i tried to just eat normal and ignore my stomach issues, i notices that i was not pooping as often as normal and poops were small and strange looking with strange colors ranging from black to yellowy pale always small or runny

6/9/08 8pm - went to emergency room this time feeling intense pressure on lower to left abdominal area and noticeable swelling overall, they did CT scan, x ray,both test showed no outstanding results. i went back home and then still felt very sick due to having to drink the large amount of iodine fluid, tried to rest and could not.

6/10/08 11am - went back to emergency room unable to urinate or move bowels, they did abdominal xray and got no outstanding results there, then they attempted to do a enima so i took the fluid and held it for 20 mins then went to the toilet, no feces came out just the clear fluid from the enema slightly yellowed. they did ultrasound of my belly and said there was feces inside, then sent me home. I bought a small feces sample from 24 hours before that was small and black and they simply threw it away saying it wasnt usefull.

6/10/08 5:30pm- went back to emergency, lower abdominal pain felt like obstruction, they did xray and got no outstanding results again. This time they gave me Golytely to help me move bowels, i drank as much of it as i could till it made me vomit i drank about 60oz of it, in between 8 oz glasses i drank i had to go to the restroom and release the watery bowel movements which seems to have more water than feces in them and came out slowly and runny not as a big movement all in all, i would get these small bowel movements every 30 minutes while drinking the go lightly solution, they did blood test results normal they say, but at this point i dont think they were even checking results it seems they were laughing at me as if i was a hypochrit.

6/11/08-6/13/08- i notice now even less active bowel movements and even further escalating of overall stomach pressure and especially can be felt on left side when laying down and throughout the day, i notice when i attempt to go poop i feel lots pressure in rectal area and there is no bowel movement or just a small pencil thin like black tarry subtance that isnt much at all.

6/13/08 - 11am go to the emergency room I wake up with abdominal pressure and attempt to go pee, then only a little
Now it has got to the point where the most I can eat a day is 1 ensure, 1 activa yogurt, several glasses of water 4-7 16 oz bottles, and a bowl of noodle soup and i avoid the hard meat parts in it. and the hole time trying do even eat that I feel very full and get discomfort in the left side of my stomach where it seems like my colon lines up and also I will hear gaseous sounds when i lay down and throughout the night when i go to bed. I feel as if my food is not digesting fast enough, I also notice when i have went to the bathroom lately

The doctor tries to convince me there is nothing wrong with my stomach and offers to do a CT scan again but i refuse since I have done one 3 days ago and had no outstanding results from it..

Now it has got to the point where the most I can eat a day is 1 ensure, 1 activa yogurt, several glasses of water 4-7 16 oz bottles, and a bowl of noodle soup and i avoid the hard meat parts in it. and the hole time trying do even eat that I feel very full and get discomfort in the left side of my stomach where it seems like my colon lines up and also I will hear gaseous sounds when i lay down and throughout the night when i go to bed. 6/13/08-6/15/08 7pm My symptoms as of right now right now are:
-slow bowel movement, small thin pencil like, black of color, tarry, smell weird, usually have a bowel movement once every 48 hours maybe.
-loss of appetite, and notice my mouth doesnt produce saliva as it used to and also always dry mouth. despite drinking alot of water.
-acid taste in mouth
-constant feeling of bloatedness and fullness before i eat and intensifies after eating, followed with being naughtous after eating, pressure mainly on left side but also bloating overall
-fatigue, lack of energy, slow reflexes, difficulty sleeping
-pressure in rectal area while trying to have bowel movement before and after and never feel satisfied of bowel movement on the rarity i have one.
-Urine has high level of bilirubin
-lower back pain
-headaches
-bloodshot eyes
-eyes very sensitive to light (migraine like aura)
-tried magnesium caltrate and enimas and they have not proved to work very well in clearing my bowels.

The only test ive had done as of now are CT scan and Abdominal x ray and ultrasound, all of which showed no need for attention in any area.

I have an ideal of what test i should get done, endoscopy of lower and upper gi, and fecal test for blood, and breath test for h pylori are my priorities, is there any other test?

also if somebody can give me a proffessional oppinion of what might be causing my problems?

Hi -many thanks for coming back to let us know your scope results; they look pretty good. How was the prep? I find that the most difficult part of scopes.  .  As you may know, Crohn's usually, but not always, starts in the terminal ileum and the presence of aphthous ulcers in that area is indeed indicative of Crohn's. I am sure the gastro took some biopsies and review by a pathologist should be able to confirm the diagnosis.  If it is Crohn's, you will be prescribed anti-inflammatories or immuno-suppressants.

For a comprehensive view of Crohn's, you may wish to google "Crohn's", scroll down until you come to the Mayo Clinic site where there is a wealth of technical info, written in layman's terms and easy to understand.

Diet alone will not bring Crohn's into remission, or keep it at bay, but can make the symptoms more manageable.  I have seen a number of hospital based dieticians over the years and they have all suggestd a low fat, low fibre, high protein, high carb diet.
I try to stay away from red meat, and eat chicken, fish, (fresh and canned) eggs and cheese for protein. For carbs I eat white rice, pasta and potatoes. Bread, even white, seems to bloat me.  Veggies can cause discomfort, especially peas, beans and corn, which are difficult to digest.  To get the minerals and nutrients I need from veggies I often make a big pot of soup: mixed root vegetables:   (onions,carrots, celery,rutabaga, turnip, potatoes,celeriac etc.) and blitz them in a food processor..  Juicing veggies (carrots, celery,beetroot, apples,with lots of tomato juice) is a good way to get what you need without heavy fibre, which can cause abdominal pain. The latter is best served cold from the refrigerator, and a litre of veg juice will last about 4 days. No more than 3 small (8 oz) glasses a day though. I also steer clear of highly spiced foods, especially anything with chillies - they give me dyspepsia. If my husband makes chicken fajitas, he does mine separately, with no chilli powder. We have almost eliminated butter from our diet (husband has stomach problems) and we use olive or sunflower oil where fat is required.

Endoscopy would be the next step.  

A colonoscopy would exclude colon cancer as well as inflammatory bowel disease like Crohn's or ulcerative colitis.

An upper endoscopy can exclude stomach cancer and evaluate for inflammation of the stomach, as well as ulcers.

Other, more specialized tests, would include sending the stool off for culture and analysis, as well as a 24-hr pH study to exclude GERD, and a gastric emptying scan to evaluate for delayed gastric emptying.  

I would see a GI physician for further evaluation.

Of course Dr Pho will be the most qualified to advise you but, from reading your posting, the need for a colonoscopy and endoscopy "leaps out".

Black tarry stools are called "melena" and I suggest that you type in this word to a google/yahoo to obtain more background.  But melena is/are formed when there is internal bleeding which is never normal and should be investigated.

If you do have Crohn's, you do need a low fibre, low fat and high protein diet...believe me, I have had Crohn's for 38 yrs and have seen many hospital dieticians.This diet is not just their opinion, but borne out by my experiences.

My recent qrtly newsletter from my Crohn's Assocn in England has just posted some IBD recipes from an IBD surgeon and writer...it is a free e-book of recipes for people with IBD.
He said" I got involved in recipes for Crohn's disease when a close family member was diagnosed. The e-book is just an illustration of the numerous possibilities of a non-elemental diet regime."Elemental regine is something like Ensure or the Fortijuice I was on for some 4 months when having FEC chemo for breast cancer and my Crohn's flared alarmingly.

i had an upper endoscopy and colonoscopy, the results found,
I have apthous ulcers in terminal ileum (possible crohn's) and minor gastritis in the stomach (checking for h. pylori).

The doctor recommended a high fiber diet but i read elsewhere I should avoid fiber if this is possibly chrons, anybody know what I should eat or avoid eating? my bowl movements seem to be hard dark and pellet like lately, but i still manage to have one bowel movement a day so im not fully constipated.

I have been eating fiber one cereal for fiber, and lean chicken for protein, is this good or should i avoid the fiber and protein and eat something else?

ok now i have a daily bowel movement, but it is hard and pebble like and darker than normal. what can this be indicating? IBS?

hey does anybody know if its ok to take reglan for my condition? does it irritate ulcers at all or anything like that?

Also think you should have a nuclear gastric emptying study to rule out Gastroparesis. Ask the GI. My wife has it and has lost 75 pounds. She only eats Carnation Instant Breakfast and then Ensure to lessen symptoms you describe.

Hi - it seems as if your doctor suspects something other than Crohn's or Ulcerative Colitis, as with these diseases a low fibre diet is normally recommended. I don' know if apthous ulcers in the terminal ileum is indicative of IBD (not IBS).  The biopsies should make a diagnosis conclusive...
Hey, 9th July - that is when I am having an upper GI endoscopy under sedation. Having a CT scan tomorrow, then results 23rd July.

Hope you will get a conclusive diagnosis on 7/9 and appropriate treatment. Shall we check back with each other? I am leaning towards either gall bladder or pancreatitis for my current problems.

my slow digestion is causing me weight loss. i am losing weight it seems cus i cant seem to eat enough to gain. without being bloatedly full too easily

can a slow digestive system prevent weight loss? Chromium Picolinate is the only thing besides fiber that ensures my business moves right along.

I am not, nor have never been big on drinking anything. How can I remember to drink if I am not thirsty?

sorry to seem redundant but is small aphtous ulcers in the terminal ileum, is that a definite indication of crohn's? or can it go away eventually? I have to wait till July 9th to see how the biopsy came out.

Hi - in my own case, constipation is very rare.......I have the opposite problem.
However, Crohn's symptoms do vary and I have read on the community crohn's forum of a lady who has Crohn's, but gets constipation.  Try googling Crohn's, particularly the Mayo Clinic site, and see if your symptoms tie up.  It can be a difficult disease to diagnose, but I understand that biopsies of suspicious, inflamed tissue is usually definitive.

Hi - Ensure should be fine and non-irritant. I wouldn't think sugar to be a problem and if you need calories, this is an easy way to get them.

When I was having FEC chemo for breast cancer 5 yrs ago, I had to stop my usual chemo, methotrexate, and had a really bad Crohn's flare. The Oncologist prescribed a nutritional drink called Fortijuice. It came directly from the hospital pharmacy to my home. I had to drink 3 cartons a day, 9 different flavours, all yucky,each had 300 calories. I lived on this for 4 months,no food at all, lost about 14 lbs, but put the weight back on after I returned to my normal diet. I don't know if Fortijuice is available in the US (if that is where you live), but there may be something similar for you to drink until you get onto appropriate medication. Ensure is a good alternative.

I have what my gastro calls "intractable Crohn's" - but he did say some people only get one or two episodes of inflammation and the disease can "burn itself out". I didn't get so lucky!

the prep was indeed very difficult for me, i used the phospho soda the night before 3 8 oz with a tablespoon oh laxative in each, my bowels were clean but not perfectly clean, in the photos of my cecum there was green particles left over from the jello i ate during the cleanse. but it was rated as a "fair prep" on the diagnosis.

So chrons does possibly go away or remits? i hope to be able to deal with this disease if it is what i indeed have.

should I avoid sugars as carbs? I've been drinking Ensure non lactose drinks to try to gain weight is this ok? or do you think it will irritate me?

went to the Gastroenterolist. had upper endoscopy / colonoscopy

Upper endoscopy indicates minor gastritis
H pylori test taken

Lower endoscopy, ulcers found in the ileum, aphtous small white ulcers, biopsy sent to pathology. Probable chrons disease.

everything else appeared normal to the report.

anybody have any oppions on this? also what should I eat or avoid eating to make this get better?

i am doing both the colonoscopy and upper endoscopy i will have to fast the day before :(

if i cannot then i will modify and only do the upper at the time, but its my best interest to do them both at the same time it will save 2000$ off the price.

In my 38+ yrs of Crohn's and preps, you don't need a clear out for upper GI endoscopy: just no food after midnight and plenty of fluids.   I am having one soon for strictures, but you do need one for colonoscopy - large colon. If you fail to clear your colon, they cannot do the test...period.  A lower endoscopy by the way is a colonoscopy...are you sure what tests you are having?

It is an absolute waste of time, and money to get a colonoscopy if you have not totally cleared your bowel..Upper endoscopy is for the small intestine.

hey in the last few days i have felt alot of pressure in my rectal area when trying to pass bowel and have not been able to move any bowel, have a scheduled upper gi endoscopy on friday, and i called today (tuesday) to urge a lower endoscopy also if possible, but heres a question if i do the osmo-prep and still fail to move bowels should i be concerned or just go and continue to get the lower endoscopy done.
