Why is CEA level increasing?

My husband had surgery 3 yrs. ago and had a right hemicolectomy. His CEA level has been good till recently.  It was 0.9 after surgery and is 2.9 now.  He smokes and has irritable bowel syndrome, diabetes and is at present having trouble with his teeth.  Gum recession is the culprit for that.  His Oncologist is concerned which is worrying him and increasing the stress he already feels.  She will retest him next month.  Are there other reasons why the CEA could be elevated or does it definitely mean that cancer has returned?
