I have this EXACT same problem. I've also had a colonoscopy that was just fine. I'd be very interested to know what could be causing this. I'm a 36 y/o male.
Can hemroids cause this? Anything else that wouldn't turn up on a colonoscopy? Any nerve or muscle issues?

Any type of colorectal lesion or mass can lead to this symptom. Excluding a mass is the first priority. This can be done via a colonoscopy.
If negative, I would see a gastroenterologist. More specialized tests can be considered, including an anorectal ultrasound or manometry tests.
