Any suggestions?

For over four months I have had a feeling (globus) sensation in my throat below the Adam's apple with and without swallowing. I also have occasional pain that moves around on the sides under the Adam's apple and can be pretty intense after eating. Sometimes but not always my throat also tightens and I feel like I am choking and it is somewhat more difficult to breathe about 15-20 mins after eating. Also, after talking for a while my voice starts to get rough. I can still eat okay without much problem swallowing.  I have seen an ENT and he did a scope above my voice box and said he didn't see anything unusual. I have also tried gerd medication with no real change. This problem is really wearing me down. Any suggestions or help would be greatly appreciated!
