Hi Doctor:

I hope that you can help me out.  When I was young I had tubes put in my ear and many ear infections.  Eight years ago at the age of twenty, my nose was broken in a fight.  After going to the hospital and having my nose put back into place, I went to another doctor.  This doctor informed me that I have large perforations in both ears.  Because I didn't have insurance at the time, I didn't have the tympanoplasty operation done, as the doctor suggested.  He also told me that I didn't have the operation performed, I would begin to lose my hearing in five years.  Four years later I got really sick and developed a horrible ringing in my right ear.  I decided to have the surgery in the hope it would go away.  One month after the surgery on my right ear, the ringing went away, but it came back after I got some water in the ear.  I've had it non-stop for 3 straight years.  Now, I've noticed an uncomfortable tightening of the muscles below the ear where the jaw and neck meet.  When I move my head I can feel the tightening even more.  I should mention that I do have TMJ.  There is a full feeling in my ear accompanied by sharp pains, which last only a couple of second before subsiding.  Would a decongestant help alleviate the feeling of fullness?  Is the TMJ causing the tightness?  If yes, why didn't it start earlier?
