Severe nosebleeds multiple times a day

It all started when i was a little kid i had frequent nosebleeds maybe once every other week or so but recently my nose bleeds have started coming on more often and way more severe.

For example today I've already had 3 nosebleeds.
My first nosebleed started spontaneously while i was crying because my head was severely hurting. My second one started after i threw up because i had a blood clot stuck in the back of my throat my third also started this way.

I have also noticed that while my nose is bleeding my head starts to hurt and i bleed severely for at least ten minutes every time also half the blood comes out of my nose and the other half comes from my mouth. Anyone have a clue what could cause this?
