Throat spasms/tightness/trouble swallowing

Thank you for answering my question in advance.  I am a 31 y/o female, healthy, with 2 kids, and am a surgical ICU R.N.  I'm hoping to get some direction for this, now, reocurring problem.  In May of 2008 I began to have a funny feeling in my throat, swallowing was a little different.  I had an endocrinologist at work feel my thyroid( he found a mass). I went for a thyroid u/s immediately(beside the point, I did have a 2.5cm complex thyroid mass with 2 subsequent Bx. with inconclusive results that they are watching).  The ultrasound hurt(strange, I know)- the tech pressed very hard into one side of my throat(can't remember which side).  Directly after that I experienced 5 months of extreme throat spasms(at the level of the hyoid bone) with my tongue also spasming.  I went to the E.R. once(I lost the ability to swallow my saliva twice b/c the spasms were so bad).  I saw my P.C.P., an E.N.T., and a G.I. M.D.- EGD negative but they all concluded it was probably reflux.  I took P.P.I.'s for 4 months with no relief. and finally after 5 months it went away.  It was worst when I laid down at night.  My airway felt like it got really small while in supine position and on my side.  I slept upright for 5 months.    So...... without warning, it all came back again 4 weeks ago.  Some nights I stay up all night long b/c if i lay down my airway almost closes.  It is not unilateral.  Sometimes when I swallow it clicks and when it is really spasms i can't burp up air.  Also, I tried taking big doses of Ibuprofen(800mg), as a way to find clues, and noticed that the spasms calm way down after 5-8 hours of the dose.  Also, any associated reflux symptoms(slight regurge after coffee/high fat foods) went away completely with the Ibuprofen(strange), leaving me only with spasms.  Like last time, the feeling never goes away- it's on a continum of uncomfortable to very scary.  I would love to have another baby but am very worried about getting pregnant while having this scary thing happening to me.  Incidently, I am not an anxious person.
