What is wrong with my ear?

Okay so I'm going to try and keep this short. I've had tinnitus for about 3 years now but in the last 6 months it has gotten increasingly louder. It's a high pitched ringing and is constant.
Another symtoms I have is whenever I turn my head to the side I can hear my heartbeat.
However I can also hear something inside my ear like a thumping noise whenever I bend over or most my head quickly this noise only happens once per movement as if there is fluid or something in my ear.
I have seen an ent and he said it doesn't look like there is anything in my ear.
I was hoping somebody could point me I  the right direction ?
