is this ruptured ear drum

hello-diaged w/middle ear infection & vertigo a few days ago; am taking jumbo antibiotics and have meds for the vertigo I can take 2x a day. This afternoon, developed a steadily worsening pain in the infected ear so I lay down w/a warm compress on it, it continued to worsen and then some clear fluid came out. That was a few hours ago-no more fluid has come out, it doesn't hurt any more, I have a dull ache @ TMJ area and feel generally ill-the ear is tender if touched. Does this require add'l dr attention? I've had ear infections before, but first time w/vertigo (yuck) and first time w/fluid leaking.
