Refusal to eat but not to be thin?

I have a friend that's male and he has THE worst appetite in my opinion.

He eats but not often. Of course because of this he's a little underweight (Probably more than a little but not too much.) and anemic.


I thought at first he might be anorexic but when I looked at the symptoms online nothing seem to fit. He's not trying to be thin in fact he could care less about body image because he always states that appearance is shallow deep and I am certain that is how he feels.


He doesn't exercise besides the average walk to someplace because of the lack transportation.


He eats with us (His friends) with no complaint but doesn't get much if he isn't hungry.


He's hardly ever hungry. From the time I spent with him he would eat JUST a crumpet one day then something else probably two days later.


He doesn't like to be told to eat either, he gets defensive when food is forced upon him.


The only thing he would eat with no problem is sweets. It's like he's always hungry for sweets because he does have a major sweet tooth. But no, he doesn't crave sweets, he just likes sweets.


I think it may have something to do with his self harm. As far as I know he doesn't do it often anymore unless he is over-bared with emotions or feels out of control of himself. He's also being diagnose with PTSD.


He and I have been close for quite some time.so I know a lot about him. I'm simply worried for his well being because his poor appetite is concerning to me. He's like a little brother I just want what's best but I can't seem to pen point the problem.


He is seeing a therapist, against his wishes. He's just doing it to lessen the concern of his friends and relatives.


What should I do?


What do you think is wrong?


I'd prefer a detailed explanation. Please don't give me any links to any eating disorders sight unless it actually contains helpful information. I can't tell you how many sites I went on and only received the basic information.
