severe nausea - PLEASE help me!! :'(

i am merely 21 years old, and have always had "weight" issues as my sister were always petite and way skinnier that me. I have never been overweight though.. However, I have always had to watch my weight and cut out carbs, etc all my life. Luckily, my mom had the same problem and so she always made sure I never over ate as to maintain a healthy weight.
now that I am older, I have taken my music career much more seriously and my producer told me I have to lose at least 25lbs. He wanted me to turn to starvation and these really hectic detox diets with lemon juice and hot spice.. and I refused so agreed to start a really good exercise routine. I exercise 4times a week, with a good workout routine so that I exercise all parts of my body and I walk everywhere that I can. I have a problem to remember to drink water because I dislike the "taste" of water.. which I think may be part of my problem.. however I do not drink juice or any alcohol or soda, etc.. just milk and water (with 5cal CrystalLite flavouring to ensure I get most of it down) and a cup of greentea with 1/2 teaspoon of honey.. I eat a lot of fruit, but don't enjoy meat so only eat a tiny serving everyday at dinner...

I have realised that sometimes I just cannot eat though. I feel sick.. when i try to force myself to eat, I throw up.. I take One-a-Day multi-vitamin because I have low blood pressure and I need the daily iron.. and 3 times a week I take a greentea supplement with a friend suggested for metabolism and skin enhancement..  the past few weeks I have found myself only ever eating at 2pm when I feel slightly hungry.. and when I eat, it's usually a small tub of apple sauce, couple carrots and perhaps a couple slices of chicken sandwich-meat. Sometimes, I'll eat left overs from the night before.. also.. I am a nanny, and I want to set a really good example for my 2 girls I look after.. the eldest is an impressionable teenager, and I dont want her picking up my new "habits".. but I cant eat.. I just get sick and nauseaus and faint and have to lay down when I eat.. On top of this, I cant stop exercising cos I'm held to a contract.. but I feel even more sick after exercising..

I tried talking to my friends about this, but they just say they wish they could do what i'm doing cos they could lose weight. YES, I want to lose weight, but in a healthy way and a healthy diet. Please help me. I dont know what's wrong with me, and I think I'm under nutritioned because I've been getting dandruff lately and bumpy skin and bad breath.. But I wash my hair 2times a week, wash my face twice a day and moisturize with good products and i brush and floss daily..! This was never a problem..! I also tend to only be able to sleep 4-5hours a night at the maximum.. I would lay in bed for 10 good hours, but not be able to sleep - even after taking 3 tylenol pm's and drinking some sleep-enhancing tea.. however, with past week I tried a few insomnia hypnotism videos, and they've been helping - so i have that part under control!!

Someone I spoke to suggested I could be pregnant.. This is not at all a possibilty.. I can guarantee that..! Also, I am not a permanent resident in the country, and so my medical insurance only covers emergencies.. and I can in no way afford to go see a doctor about this.

Reallt hope you can help me.. I just had lunch (1slice wholewheat toast with chicken salad and lettuce and a handful of grapes and a cup of water) and I am so sick to the stomache.. Feels like I'm going to throw it all up but pass-out at the same time.. WHAT'S WRONG WITH ME!!?

p.s. Sorry about the non-american spelling.. I was brought up in SA, so was taught to spell differently.. :)

Hi! Severe nausea, repeatedly feeling hungry, trouble sleeping can be due to exacerbation of the GERD or gastric reflux.  It could also be hiatal hernia. Treatment is a combination of drugs to reduce the acid and lifestyle changes. You will need to take a combination of medications (under medical supervision) like a proton pump inhibitor such as omeprazole, lansoprazole or pantoprazole empty stomach in the morning and an antacid gel after meals for complete relief. Possibility of H pylori infection too should be looked into by a carbon urea breath test and a combination antibiotic tried. Many a times a persisting H pylori infection can be the cause behind acidity not responding to treatment.
Life style changes that will help include: Avoid heavy meals and eat frequent small meals. Avoid too much of caffeine, tea, smoking, fried food and drinks both alcoholic and non alcoholic fuzzy ones. Avoid heavy exercises within 4 hours of a heavy meal. Raise the head end of the bed by pillows to 30 degrees. Avoid lying down for least 2 hours after food. Maybe these tips will help you.
Commonest cause of chronic nausea is acidity. Liver and kidney dysfunction is another cause. Hence kidney and liver function tests should be done. Other causes are diabetes and hypothyroidism. Electrolyte imbalance too can cause nausea. IBS, gluten or lactose intolerance, parasites in gut too can be the cause. A stool test should be done for worms, giardiasis etc. Chronic hepatitis or chronic infection of the gall bladder are the other possibilities. Also if nothing is found then food challenge test to detect food intolerance should be done. Keep a food diary and see if there is a co-relation between what you eat and symptoms that appear. Chronic constipation, sedentary life style building up gas in the gut, inflammatory bowel disease, food rich in sulfites (molasses, dry powder, pickles, tinned shrimp, cookies, crackers, and readymade pie dough), cabbage, raddish etc also cause bloating and nausea.
Please consult your PCP for primary examination followed by proper referral.
