Endometrioma on Left Ovary

I have been suffereing from regular occuring severe pain from endometriosis for almost 20 years and recently the doctor discovered that I have a 4 cm endometrioma on my left ovary using an internal scan.  He is monitoring it, I have to return in 3 months.  I am now in peri-menopause, in my mid forties and am wondering if surgery is necessary, should I have just the ovary removed or should I have a total hysterectomy.  The only reason I've been holding out on the hysterectomy is over concern for the loss of estrogen at such a young age plus the fact that there were always treatments that gave me relief for months at a time.  Now, I can't stand the side-effects from those treatments so I'm taking pain killers which do not keeep the disease from interfering with my life and of course, I'm older.  On the other hand, I've heard of women in their mid-forties who are in peri-menopause and have an ovary removed (because of some type of growth) and as a result, they go into menopause.  Is this a common occurance?

Have you had prior surgery for endo?

I have Stage IV endo and major scar tissue. My surgeon was the best and excised (best method) all of it including the endo that was covering my bowels, ureters, and bladder.  

My endo is back however 5 yrs later but what I have found its the same areas again. My left ovary has attached itself to my uterus and because of all the scar tissue my uterus is retroflexed and antevered (curled up like a snail) and pushing against my colon.  

What I might do is have the ovary removed because it seems to be the problem all the time.  

Having a total Hyster is something I have thought about over the last 8yrs. I have had endo over 20yrs as well but was only diagnosed 4yrs ago.  

I am not sure if doing that is the right thing because it doesnt remove the endo you can still get it back.  

This disease is very frustrating and complicated. No matter what you decide to do everyone responds differently.

I am 27yrs old, and I just had my left ovary and fallopian tube removed and they did a biopsy on it and found out that they were filled with endometriosis/endometrioma,  I have no children but my doctor I see told me if he had his way he would actually do a hysterectomy but he knows how bad I just want to have 2 kids that he gave me a five year window and when that is up I will have to have a total hysterectomy done.  I pray alot to God that he will bless me, I just had my 3rd Laparoscopy done September 14th,2011 which was to remove my ovary and fallopian tube but having a laparoscopy is not bad at all, If you have an ovary thats ALOT bigger in size like mine was it would be best to have it removed, to top it off my left ovary wasn't even in the right spot where it should be and was causing me ALOT of pain but now that its removed I am hoping to be able to get pregnant within the next month or two and I am not suffering from the ovary pain I had before surgery but I have minor pain from the incisions healing but its not that bad at all

Hi I am also in the same boat you are. I am 25 and will be 26 yrs old next month by God's grace. I have several cyst ruptured this year and I might have endometrosis, I am too getting a lap done but don't know the procedures. I was thinking about getting a hysterectonomy done but I want one more child before I do that. but the doc says he will have to go in and look around and see what is going on inside of me :(. I am terrified! I don't know what to expect. My mom or both of my sisters don't have this problem so now I am thinking what went wrong with me? I feel cursed most times anyway. My life has been nothing but trials and tribulations but to GOD be the GLORY! God must see something in me to take me down a road like this. I am grateful to be alive and I thank God  for my son that I do alrady have. God is good anyhow through it all! let me know how you are doing ok? thanks

you should opt for a total hysterectomy.  You are at the right age of having it since an ovarian cyst or endometrioma on an ovary at your age won't do you any good and can turn into cancer or affect the fallopian tube.  I just found out I have an enlarged left ovary and was diagnosed with endo last year thru a lap.  I'm only 26 years old and wish to have a baby next year.  I'm scared of having another lap but worried that there is a small chance of ovarian cancer although my doctors told me that they've never seen anyone at my age to have this cancer.  Anyway, you are going to have menopause anytime soon now and will have to suffer from the menopausal problems anyhow.  But I learn that hysterectomy can at least reduce your risk of many gynecological cancer in the future especially you have a history of endo.  Good luck and stay strong!
