Enlarged Ovary and Thick Uterine Lining

I have been diagnosed with PCOS and CAH (congenital adrenal hyperplasia) since 1999/2000. Both conditions have been uncontrolled for approximately 7 years--during which time my periods occurred once/twice a year. In 2007, my doc went in to scrape my uteruse, remove some polyps, and scar tissue in my abdomen that had attached to my ovaries. I had one period immediately following the procedure and only 1 or 2 since (I am working with an Endocrinologist and OB/GYN). As of a month ago, my uterine lining was incredibly thick and I have an enlarged ovary (4 cysts on the normal ovary; none on the enlarged ovary). I was started on Yaz to kick-start a period (for about eight months I have had continual bleeding but not a period--like a drippy faucet, but with extreme pain as well). My OB/GYN said he would like to biopsy in one month if my lining has not significantly reduced at the end of taking one month of Yaz. However, two weeks into the pills, I started bleeding heavily and passing some uterine lining. Here are my questions: (1) I have passed uterine lining before and it has always been tissue-y and dark red in color. This time I have passed material that has the same consistency of past uterine lining, but parts are also opaque and white. What does this mean? With my doctor wanting to do a biospy to rule out uterine cancer, I am a little scared. (2) Should I really be concerned that I might have cancer with the thick endometrial lining and the enlarged ovary? I am in pain most of the time (5 out of 7 days at least) and even when the pain isn't severe, I still ache something awful. (3) I am continuing the Yaz until the pack is done, but my endocrinologist doctor doesn't want me to start another pack. He believes my lining is so thick that there is no place for more build-up. (4) Do the instances of cancer increase if I have both an enlarged ovary and a thickened uterine lining? (My OB/GYN is concerned with how thick the lining is). A thickened endometrium could be due to endometrail polyps or hormone levels being out of wack.
