endometriosis

hai I have bilateral ovarian cyst, now iam on birth control pills femilon, but I have left abdomen cramping and brown discharge. what is the reason for that. is it that the cyst is resolving and debris moving out. does ca 125 value increases in endometriosis. how we can manage pain. has anyone experienced continuous pain sympotoms .what is the diet care we should take.

So to start off with have you been diagnosed already with Endometriosis or have they suspected its endo?

The ovarian cyst you have is that an Endometrioma?

Brown blood is old blood and your right it just wants to come out and that happens a lot when people have cysts.

The BCP is to help to see if it will shrink the cyst. I assume they have told you that you will go for a follow up ultrasound to see if the pill is working?

The CA-125 is used to check for inflammation but is not necessarily an Endo marker. It is used to check for Cancer, adenomyosis, Endo and various other inflammatory diseases. That being said I had mine done 3 times because they suspected cancer. The first time it was 37 on my period than 78 during ovulation than under 35 another time of the month.

Managing pain would be to excise all the Endo from the root. Excise is cutting it out where laser only gets the surface.

You need to find an Endometriosis specialist who excises. Once you have excised all the endo most patients find they have 80% relief but that also means staying on something continuous after that like the BCP continuously to stop periods hence stop the pain. That is why I was wondering if you had surgery to diagnose this yet.

The diet will not stop Endo from growing neither will the pill but it helps elevate the symptoms.

The Endo diet is no processed foods, no dairy, No gluten, no sugar and no red meat. That is the basics of it. Hard to follow but if you can try it might give you more energy and elevate pain symptoms.

I totally understand why you are doing it. Let me put your mind at ease. If you have never heard of Dr.Redwine google him after this post he is one of the most famous surgeons for this disease and has 30 yrs experiences in research and surgeries for Endometriosis with having also done 3000 Endo surgeries. He has many books out and he is my go to for questions, He is retired now so he has more time to help other people with their questions but seems to only answer questions that are not repetitive. Here is his response to me when I asked him about the CA-125 blood testing for Endo.

CA 125 can be elevated with fibroids, adenomyosis, ovarian cancer, infection, adhesions, endometriosis, and for no apparent reason. It can also be in the normal range with those conditions. Thus the CA 125 is helpful in narrowing a list of seven possible causes of problems down to the same seven possible causes of problems. If a patient has endometriosis only, an elevated CA 125 level usually is associated with endometrioma cysts and invasive disease. Around the time of ovulation, the estrogen level is at its highest, so endometriosis would be a little more stimulated to be problematic at that time, which might cause the CA 125 to go up as well. I did not find the CA 125 very helpful at all and I never ordered it routinely in my patients.

thank your for your kind reply. regarding the ca 125 test the latest value is 92. six month back it was 172 I was very scared . every month i used to repeat. it was coming down in july it is 92. what do you feel about the ca values. it worries me a lot. I do it everymonth. my gynec scolds me.what is your opinion madam

Brown blood-Old blood is pretty common with Endo. I wouldn't worry too much as it has to do with the cysts. It could be left over shedding from the uterus or debris from the cysts that have the Endometriomas.

I also wanted to add that if they have said they are chocolate cysts 99.9% of those are NOT malignant. I think you are dealing with Endo and Fibroids. October isnt that far away so I think it should be fine if you can bare the pain.

However I would keep getting the ultrasounds done and stay on the pill to make sure they don't grow any bigger.

Try not to worry. I know its hard. In 2006 I came here asking in the Ovarian Cancer form about a 10 cm complex cyst I had. I was so panicked and couldnt sleep thinking it was cancer.

What was your CA-125 result? If you haven't had one I would get that to ease your mind.

If you want my honest opinion I will tell you to not have the hysterectomy.

I would find an Endometriosis surgeon who knows how to cut this disease out as well as fibroid's. After the surgery continue with pill that you are taking to stop your periods hence stopping ovulation of any sort and hopefully help with pain.

I would go this route first as you have not yet. Having a hysterectomy is the last step you take after doing this first. If this does not help then that choice would ultimately be yours of course and if I was in your position I might do that too.

First I ask that try an excision surgery if possible.

Lets say you do the hyster and its done by laser and they remove everything. The Endo was not excised and it wasn't taken out properly by the root so the chances of it coming back are really high and you will probably not get the relief that you expected, ultimately leading to surgery after surgery.

Excision is Key when combating this disease I can't express that enough and if you have 2 endometriomas of that size then you probably have severe disease and if it was me I would want it excised. You will see the huge difference.  When someone has Stage IV which I can only assume you do due to the uterus tilt and the large endometriomas the disease is probably on your bowels and bladder. Do you have digestive issues or bowel, bladder pain?  

Fibroids can be removed at the time of Excision surgery as well.

I can help you find a good surgeon. If you are going to battle this you need to do it right the first time. I am telling you this because I care and because I have seen so many women who have not done excision and the amount of surgeries and damage that gets done with laser is very high.

Do you have coverage for this type of surgery with an Endo specialist? I would find this out first.

I was first diagnosed by left ovarian cyst size 2.3mm. I has secondary infertility problem and my menses was very painful. We planned for the second child very late for 10 years due to family problem and my PhD work. God has gifted me with a loving daughter who is now 12 years. In the year, 2011 we went for the fertility treatment. Doc gave me three months of Birth control pills (Novelon) which showed a resolving cyst. After that I was treated with Siphene for ovulation. But I did not conceive, instead I developed a right ovarian cyst. After 6 months I went again for the fertility treatment (IUI) , because my hubby had decreased sperm motility. But it was unsuccessful. 6 months back I again went for the IUI. During that time my left cyst was resolved and my right cyst was there. In the IUI process I was treated with siphene (ovarian stilmulation drug) . I t was unsuccessful. My gynaec  said it is endometriosis , if I conceive my endometriosis will disappear. But to be frank everytime I do the fertility treatment my cyst becomes worst . I think due to the hyperstimulation of the cystic ovaries. Now I have decided not to go to second child. Ia planning for hysterectomy after so much suffering
       I have regular menstrual cycle of 28 days, bleeding normal. I have severe menstrual cramps first and the previous day of my periods. I cannot do my daily activities. I should apply two days leave each month. I went for ultrasound scan last month (5th june 2013) -13th day of my menstrual cycle. Report is as follows
Uterus-  is anteverted. Multiple fibroids are seen in the uterus. Size: 6.4x4.4cm
Uterine endometrial thickness is 7mm
Right Ovary: Multilocular echoluscent lesion with debris is seen Size: 6.4x4.4cm
Left Ovary: Multilocular echoluscent lesion with debris is seen Size: 5.8x 4.8cm
Pouch of Douglas is free
Impression:  MULTIPLE  SEEDLING FIBROIDS IN THE UTERUS.BILATERAL OVARIAN CYSTS WITH DEBRIS- CHOCLATE CYST.
           This month from 4th day I am taking birth control pills (FEMILON). I had left side pain for 15 days even though I was on BCP, from past two days the pain has disappeared but lot of brown discharge. Is it the sign of resolving cyst. Hope my cyst size reduces. So iam worried. Shall I go for hysterectomy immediately  or I can wait till October because my daughter has vacation that time. Please tell me your suggestion. Im  worried about malignancy so I don’t get sleep in the night. Please help me by telling your valuable suggestions, I mean it a lot and respect it also . Please pray for me also.

The only way to confirm Endo by ultrasound would be if there was an endometrioma. If there isn't Endo will not show up otherwise. So do you know if that cyst was an Endometrioma?

For 100% diagnoses of Endo it needs to be done my lap surgery.

When I was on the pill before my surgery it did nothing for the Endo but it helped my hormones regulate which was just as important.

You really need to find an Endometriosis specialist that excises that is the only way you will have some relief. It seems as thought they have told you they think its endo and are doing nothing about it and you can't live that way.

Push for a surgery. Do it right the first time and get the best of the best. You can tell me where you live and I can help you find the best and ask for a referral to the surgeon.

A hysterectomy will not cure endo and can make it worse for you. Your organs can start dropping, sex would be more painful, Chance of early onset dementia and osteoporosis. Trust me I have research this way to go for a long time and I understand where you are at but you haven't had a lap to remove any of it.

The pain you are feeling is probably a cyst. The pill could reduce it a bit but if its an endometrioma then it wont. You need it removed.

Do you have the ultrasound report? What did it say on it?

thank you so much for your kind reply it meant me a lot. I  had been diagnosed of endometrisosis earlier , by ultra sound. iam worrying about continuous dull ache in the left lower abdomen . iam taking pracetomol in a gap of 12 hours . I don't knw how many days it will take for the birthcontrol pills to bring down the cyst. can I go for hysterectomy. iam fed of pain
