Well, like I said it could be the chronic sore throat cause by Fusobacterium as In my post. Some strep that cause strep throat can be resistant to Azithromycin. It depends on if they did a culture and antibiotic sensitivity test or threw an antibiotic at you. Which is what doctors tend to do. Most streps are still sensitive to penicillin though, but some strains have become resistant or not as sensitive anymore. But if you had strep throat you would have a high fever and usually pus in the throat (but not always). If you have reflux it is possible that there is some erosion going on. Do you know you have GERD?  If so you can take Zantac or one of the other meds to try to control it and see if that helps. If you are concerned about cancer you cancer you would have to see an ENT for them check you . But you don't want to. I still think it is bacterial or viral or possibly GERD.

Hi and so sorry you have this problem. Do you have pus in your throat and/o a high fever? There is an anaerobic (grows without oxygen) bacteria that can be in your throat normally or can cause chronic sore throat.  It is called Fusobacterium necrophorum. It isn't always seen in the throat and the regular antibiotics wouldn't work on it  You may need metronidazole. That is if you are not allergic to it. You may ask your doctor on the phone if they would allow you to try it or an antibiotic that he/she finds suitable to treat this anaerobe. They can e prescribe it to the pharmacy. Fusobacterium necrophorum has been linked to Lemierre syndrome,a disease characterized by peritonsillar abscesses and thrombophlebitis of the jugular veins after a bout of acute sore throat. I don't know for sure that you have Fusobacteium. the only way is to culture it out. But since you don't want to go to the doctor I am telling you that this bug can cause chronic sore throat and doesn't respond to the drugs that they usually give you for strep throat. So it is possible that you could have it. That is my professional opinion as a clinical microbiologist  The only other thing is are your lymph nodes swollen? Could these be what you feel when you swallow?  You may have had the original strep throat and then your lymph nodes swelled and that is hat you feel? That with some residual pain from the sore throat?  I don't know how bad your throat feels. But those are my two thoughts on this.
