Negligence during an EGD

What is considered medical negligence regarding an esophageal perforation during an EGD? Would it be just one perforation or multiple and what qualifies for multiple perforations? For example a bruise could be microscopic perforations very close together but negligence might be 3-4 perforations 0.5mm long separated by 0.3mm. Please respond only if you are an esophageal or gastrointestinal doctor or expert. Please site your expertise. Thanks
