The best advice I can give is that you speak directly with your doctor. Do you have a personal trainer available to you at your gym? If you were at my gym I would be able to help, but not knowing your full history makes it difficult.

The first thing I can think of is that you are not raising your heartrate enough during your exercise. Usually I tell my clients to stay at their target heart rate at least 40-45 minutes a day continuasly to see good results. This may not be possible with your limitations though.
